How it's calculated

Ratio = waist circumference ÷ height (same units)

The WHO/Ashwell guideline: keep your waist to less than half your height (ratio below 0.5) for the lowest metabolic risk band.

Why belly fat matters more than the total number

Not all fat behaves the same way in the body. The fat sitting around your abdomen and organs, visceral fat, is metabolically active in a way that pushes up risk for type 2 diabetes, heart disease, and fatty liver, well more than fat stored elsewhere does. Two people can weigh exactly the same, share the same BMI even, and still carry very different amounts of visceral fat, with correspondingly different risk.

This ratio works because it measures the region that actually matters for that risk, without needing any lab equipment. That's a big part of why it keeps showing up alongside BMI, or instead of it, in newer clinical and public health guidance.
